The other week I found an in-depth article on a fatal 1997 accident involving the failure of an Octopus ride. The article appears in the "Journal of Failure Analysis and Prevention".
The journal & the report are copyrighted and I had to do a lot of permission requesting. With some success, I got permission to place it on the LTC website.
